Please don't send your family members here. There is a reason they are a one star facility on CMS Five Star Rating System. Their health inspection is a one star as well as quality measures. If you don't want to rely on CMS, take it from me. There is one wonderful human being here - Patrice, but I didn't speak to her until just before I moved my mom from this facility. My mom was private pay here for a couple years, and when her private pay money ran out, I had to apply for Medicaid for her. The business office gave me incorrect information regarding Medicaid, so she was denied at first. Then I had to go through an attorney to get approval. During this process, I provided the staff (administrator, business office, social worker) with all of the attorney's information if they needed updates, etc. The administrator accused me of lying about applying for Medicaid, stating that the attorney's office did not exist, though it was easy to find on google. She then had her social worker email the ombudsman saying that I was being untruthful. A total joke since I had been in constant contact with the Ombudsman who confirmed a quick google search showed the office and phone number, and confirmed she knew I provided all of this information before. The administrator would call me and other family members while Medicaid was pending, threatening to discharge my mom, asking when we could pick her up, etc. I had to tell the administrator MULTIPLE times the same information, confirming she does not keep records or has a potential brain tumor. Its hard to contact anyone here because they don't like to pick up the phone. My mom's room was changed without me being called. Her meds were changed without me being called. They fail to have consistent care plan meetings, which is important as we don't live in the same state. During care plan meetings, only a few people would be in attendance, not the whole team, and I would get minimal information on my mother's condition, when she has Alzheimer's and cant speak for herself. Just a few days prior to my mother's move to a different facility, a nurse called telling me that my mother was unresponsive, wouldn't wake up, etc. They were going to hold her for observation and let me know updates about her condition. They believed it was a seizure like she had before. I never heard anything the rest of the day, so I called the next morning for an update. The nurse this day was confused, stating that my mother was sitting right in front of her, being her normal self, awake, eating, etc. She went through the notes in my moms chart and there was NOTHING stating that she had been unresponsive, that I was called, etc. NOTHING. I have too many examples of poor care and poor treatment than I can count.
I've had the local ombudsman involved multiple times during my mother's stay, as well as having to file complaints through the health dept. They are truly not worth the hassle. If you love your family member, this is not the place for them.

I want to add that I noticed other reviews where the company apologizes and wants more information, so I want to add that I called NHS management to speak to the management team directly about the issues here, explained everything going on, was told I would be called back, but nobody ever followed up.

My sister was recently in the Wetumpka facility before she passed away. During her stay our family was mostly pleased with her care. The administrative staff was kind and helpful in all areas. The nurses and CNAs for the most part were attentive to her needs and tried to keep her as comfortable as possible. Wesley, one of the floor nurses, was exceptional in his knowledge and devotion to his patients. He was instrumental in seeing to it that my sister was afforded as much comfort as possible. CNAs Keisha and Sommer could not have been more efficient in their job and caring in their attention to my sister. These three in particular went above and beyond in taking care of her and we truly appreciate it. As the end neared for my sister the entire staff showed professionalism and compassion and we are deeply greatful.

About this data

How to read this page

Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.

Where this data comes from

All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
