TERRIBLE. please do not put any family members here. had my mother in there the workers could care less and are so rude. they throw the food to the seniors like dogs. workers are always on their phones never giving the elders any attention and give them unwanted attitude. my mother never had her diaper changed.

Worst facility ever created. I have no problem sharing my story. Both parents went there...mom failed to thrive, dad had a broken neck and no one knew how to care for him or even know how to put a neck bracebon correctly. We had to call 911 to get him out of there.

After my car accident and 4 days in the hospital I was transferred to Fallbrook Skilled Nursing. I’d read other reviews and was skeptical but one review mentioned that the physical therapists were great and that was the reason I was going so chose them. The admissions woman, Janet, even called me beforehand to answer any questions I had. I was SO pleasantly surprised at how good all of the staff were! Every one of them I came in contact with was professional and had great attitudes. They are a bit understaffed but never let that issue control their day. There are wonderful people working there that let me know they were there to HELP me get better. The physical & occupational therapists were great. Special shoutout to Carmen, my CNA, who has a heart of gold and the room brightened every day she was at work. The food was also very good-the cooks do a great job!

I was transferred to Fallbrook Skilled Nursing from Rancho Springs hospital in Murrieta in February 2018. When I arrived there I was pretty much paralyzed , only able to move my arms very little . I was at Fallbrook for just shy of 4 months before being discharged. I had excellent care by everyone in the facility , with a special shout out to Brent , the head therapist.( since my illness I have pretty much recovered)

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
